Organizational Employment Services (OES) Fact Sheet
Population Served
Services are provided to individuals who have been found eligible to receive Adult Services through the Summit DD Board and the State of Ohio and have a desire to work in a center based environment, participate in habilitation activities or a combination of both services.
Settings
Services are provided in Summit DD facilities located around Summit County. Individuals are encouraged to receive services in the facility located near the residence and in their community.
Hours of Services
Services are typically provided between the hours of 8:15 a.m. and 4:15 p.m. The day program participant is typically at the center for 6 hours a day. However, this schedule may be modified based on the individual’s needs.
Days of Services
Services are provided daily Monday through Friday.
Frequency of Services
Frequency of services is discussed with the individual and through his/her team to determine how frequently the individual would like to attend the day program.
Payer Sources
Services are funded through a combination of funding sources including Federal, State and Local Levy dollars.
Fees
There are no out-of-pocket fees to participate in facility based programs at this time.
Referral Sources
Anyone may make a referral for Summit DD Services by contacting the Intake Department. Once an individual is eligible for services, a Service and Support Administration Coordinator is assigned and will assist the individual with choosing a provider for day program.
Specific Services Offered-Organizational Employment Services (OES)
Adult Day Supports: These are non-vocational day services needed to assure the individual is functioning to his/her best abilities. Services consist of assessing interests, supports and services provided at the centers. Supporting and supervising individuals in the area of personal care include attending to their personal hygiene, eating
assistance, communication, mobility and restroom assistance which will ensure an individual's ability to experience and participate in community life.
Vocational Habilitation: These are services designed to teach and reinforce concepts related to work, including responsibility, attendance, task completion, problem solving, social interaction, motor skill development and safety. Contract piece work is the vehicle to learning these skills and earning a paycheck. A combination of both programs is available. An individual must be able to participate in vocational habilitation a minimum of 10 hours per week.
Entry Criteria
Person Served/Individual shall:
Meet Ohio Department of Developmental Disabilities Adult Services eligibility requirements.
Must be able to meet health and safety needs of the individual and others being served in the day program.
Must meet the attendance requirements.
Transition Criteria
Persons needing to transition to other levels of care/services as well as transition to other programs and services are based on the individual’s needs and is discussed and recommended through the individual’s service coordinator and team.
Exit Criteria
Persons Served: Is no longer eligible for Ohio Department of Developmental Disabilities Adult Services. Transfers to another alternative program/provider other than Summit DD.
Is unable to maintain entrance criteria: May include excessive absenteeism, refusal to accept requested/needed programs/services or poses a threat to self and/or others which programs/services are unable to address at the day program.
