Sizzle Video Shows Summit DD’s Personality

Summit DD launched its new brand and website this year. Our brand is a reflection of who we are. It was developed by those who know us best: individuals we support, families, staff, and the community. We don’t want to stop there. We want to give those who don’t yet know us a peek at who we are. And what better way to show our personality than through video?

For those who already know us, you’ll see the amazing people who inspire us, the caring staff who live the Summit DD mission, and the innovative supports that keep us moving forward. To those who don’t know us yet, we invite you to check out this video. You’ll see that we’re the community resource connecting families and individuals with developmental disabilities to the right supports to live their dreams.  We think you’ll agree, Summit DD is more than meets the eye and we’re just getting started!

Summit DD Launches New Brand, Website

After 47 years of supporting adults and children with developmental disabilities in Summit County, Summit County Board of Developmental Disabilities announces its new logo, brand and website featuring an intuitive, user-friendly navigation with additional resources for families and citizens.

“It was important to the Agency and the Board that we collect input from our stakeholders during the rebranding process”, stated Superintendent John Trunk. “We wanted to find out if our customers agreed that a fresh brand was needed, what visual images would portray the value that Summit DD brings to their lives.”

Eligibility

Determining Eligibility

Who can make a referral?

Referrals can be made by individuals, parents, physicians, teachers or anyone who believes that a person residing in Summit County may benefit from and qualify for services from Summit DD. The Agency then gets consent from the individual, parent or guardian to begin the eligibility process.

What is the criteria for eligibility?

Eligibility for services is based on a number of factors and evaluations that are specific to a person’s age.

Birth through Age 2

All referrals for children under the age of three are made through Ohio (Part C) Early Intervention (Help Me Grow). Summit DD works in partnership with Ohio Early Intervention to provide developmental evaluations for children who have a suspected delay or who have a medical condition which has a high probability of resulting in a developmental delay.

Children will have their eligibility re-determined at age 3 for continued services.

Learn more about Summit DD's Early Intervention Services or contact Help Me Grow to begin the eligibility process.

Ages 3 to 5

Children, ages three to five, are eligible for support if they have an Individual Education Plan (IEP).

Children will have their eligibility re-determined at age 6 for continued services.

Ages 6 to 15

Children, ages six to 15, are eligible for Summit DD services if they have a documented
history of a life-long developmental disability and significant functional limitations in at least three areas of the Children’s Ohio Eligibility Determination Instrument (COEDI).

Eligibility is re-determined at age 16 for continued services.

If you or your child is found eligible you be assigned an (SSA) Service and Support administrator or an (RSS) Referral and Support Specialist. One of these staff will assist you or your child with your current needs and future planning. If you or your child are not found eligible for services, our team will continue to work with you and help identify other community resources that may be available to you. You also have the right to due process to appeal the decision if services are reduced, denied or terminated.
