Services for Adults
Adults face many decisions in life, such as where to work, where to live, what interests to pursue, which friends to spend time with, or how to make ends meet on a limited budget. Adults with developmental disabilities are no different. At Summit DD, we use a person-centered planning approach to help you design an Individual Service Plan, or ISP, that reflects your unique goals, interests, and needs.
Your SSA (Service and Support Administrator) or RSS (Referral and Support Specialist) will work with you to connect with providers, community resources, and natural supports that can help you thrive. Summit DD partners with a wide network of quality service providers throughout Summit County. Your SSA will also stay connected to ensure your services are monitored, well-coordinated and continuously meet your evolving needs.
Available services may include:
- Center-Based Day programs
- Community Employment support
- Transportation options
- Residential support
- Social, Recreation and Community Activities
- DODD Online ICF Search Tool (DODD Electronic ICF Informational Booklet)
