Adults face many decisions in life, such as where to work, where to live, what interests to pursue, or how to make choices on a limited budget. Summit DD is here to support you as you navigate the many decisions of life.
What is Person-Centered Service Coordination?
Service Coordinators, such as Service and Support Administrators (SSAs) and Referral and Support Specialists (RSSs), help you identify your needs, understand your options, and connect you with the right resources and supports.
Once you are found eligible for Summit DD services, your service coordinator will listen to your goals and work with you to create an Individual Service Plan (ISP). Your ISP uses a person-centered planning approach to outline what is important to you and the supports you need.

Your service coordinator is your primary point of coordination and will help you navigate services and supports. They will:
- Develop your ISP using person-centered planning to identify your goals, preferences, and support needs.
- Connect you with providers and community resources that can help meet your needs so you can achieve your goals.
- Develop a service budget based on your assessed needs and available resources.
- Coordinate and monitor services to ensure the supports outlined in your ISP are being provided and help address health and safety needs.
Service coordination is provided at no cost to people who are eligible for Summit DD services. Your service coordinator is here to help you understand your choices, navigate available supports, and work toward your goals.
