FAQs: Release of Information Requests

From time to time, we receive record requests for people served by Summit DD. Below are guidelines regarding who can request information and what information can be requested.

What personal records information about an individual can be requested from Summit DD?

Any Summit DD records can be requested, including ISPs, C/OEDI and assessments completed by Summit DD staff to name a few. Summit DD requires written requests to provide copies of the records or reports. Please note  that Summit DD may NOT be able to send documents or records that were created by another organization, such as school records, medical/psychological reports or records pertaining to MUI investigations.

How do I make a records request?

To request records or personal documentation, please complete and return the Summit DD Release of Information Form. Be sure to fill out the entire form with as much detail as possible to help expedite the process. (Then see Where do I submit my records request? question for email or mailing information.)

Who can make a records request?

Due to strict HIPAA regulations, records can only be requested by the eligible person, their guardian or their parent (for minor children).  An individual may request that confidential information be conveyed by Summit DD to the individual through alternative means or alternative locations.

When will I receive the information requested?

Once an eligible individual submits a records request, it typically takes 5-10 business days. If there are any questions regarding your request, you will be contacted.

How are the records sent?

Records can be mailed, emailed or faxed. Records will be sent as specified on the Release of Information Form.
